We now require a credit card to book an initial visit. Our system, Square, is very secure. If you do not have a credit card, please call the office to schedule. We appreciate your understanding!

Please print off our intake form         to fill out prior to your appointment to save time once you arrive! If you are unable to print and complete the paperwork, please come to your appointment 30 minutes early.

If you are attempting to book a new patient exam and treatment and you are using Worker's Compensation, Personal Injury, or Auto claim, please call the office to schedule your appointment.

We are no longer in network with GHP family (medical assistance). We are still in network with all other Geisinger plans.
